What can make up the cost of a SAP program
- SAP evaluations. Published 2025 and 2026 provider prices vary widely. Examples we reviewed range from about $300 to $700 for an initial and follow-up evaluation package, with some providers charging separately.
- Education. Published prices for basic education programs commonly start around $150 and can reach several hundred dollars, depending on the program recommended.
- Treatment. Counseling, intensive outpatient care or residential treatment can cost substantially more. Published provider examples range from hundreds or low thousands of dollars for some outpatient programs to many thousands for intensive or residential care. Your SAP determines what is clinically appropriate, so treatment cost cannot be known in advance.
- Return-to-duty testing. Publicly advertised drug testing prices we reviewed are commonly around $50 to $120. Charges vary by collection site and service arrangement.
- Follow-up testing. The SAP must prescribe at least six unannounced follow-up tests in the first 12 months of safety-sensitive duty. Per-test costs vary and can be similar to other DOT testing charges. The driver must not be given the actual testing schedule.
- Travel and time away from work. These are not DOT fees, but they may affect your personal cost.
These figures are market examples, not federal rates. DOT does not set SAP, education, treatment or testing prices.
How to compare costs before you book
Ask for the total price and what it includes before paying. Useful questions include:
- Does the quoted SAP fee include both the initial and follow-up evaluations?
- Does it include the SAP reports required by Part 40 and, for FMCSA drivers, the required Clearinghouse reporting?
- Are there separate administrative, rescheduling or documentation charges?
- Does the SAP offer a permitted remote video evaluation that could reduce travel?
- Who will pay for the return-to-duty and later follow-up testing?
- Will a health plan, employee assistance program or other benefit cover any recommended treatment?
Is there a free DOT SAP program?
DOT does not require an employer to pay for a SAP evaluation, education or treatment. Under 49 CFR 40.289, payment for SAP evaluations and services is left for employers and employees to decide and may also be affected by labor agreements or health benefits. Some employers or benefit plans may cover part of the cost. Read more in Is there a free DOT SAP program?
Price does not change the SAP's clinical recommendation
The SAP must evaluate each person individually and base the education or treatment recommendation on that clinical assessment. An employee cannot obtain a second SAP evaluation simply because they disagree with the first SAP's recommendation. 49 CFR 40.295 prohibits seeking a second SAP for a different recommendation, although the original SAP may modify a recommendation when new or additional information justifies it under 49 CFR 40.297.
