Our SAP quote service
DOT SAP Services
Clear information to help you plan your return-to-duty process.
A simple request. No payment to submit. No obligation to book.
Where do you need an SAP?
What we do
We help you ask.
You decide who to choose.
One place to request quotes from SAP providers.
Enter your ZIP and contact details, with appointment preferences if you want to add them. After human review, if a suitable match is available, up to three relevant participating providers may contact you with their fees and availability.
Our role is to help with the introduction. Your chosen provider is responsible for their credentials, evaluation, recommendations, fees and service.
Compare with confidence
Ask what the quote includes.
A lower headline price does not always tell you the full cost.
| What to compare | What to ask the provider |
|---|---|
| SAP qualifications | Do you meet the applicable DOT SAP qualification requirements, and can you practice where I am located? |
| Evaluation fees | Does the quote include the initial evaluation, follow-up evaluation and required reports? |
| Appointments | What is your earliest appointment? Is a remote evaluation appropriate and permitted for my location? |
| Additional costs | Are education, treatment, testing or other services charged separately? |
| Payment & cancellation | What payment terms, rescheduling rules and cancellation charges apply? |
Return to duty
Know the steps
beyond the quote.
Each person’s recommendations and timing are determined individually.
The overview here describes the general DOT process. Your SAP and employer will explain the requirements that apply to your situation.
Submitting a quote request does not begin a clinical evaluation, satisfy a DOT requirement or authorize you to perform safety-sensitive duties.
Read the official DOT SAP guidanceInitial SAP evaluation
A qualified SAP evaluates your situation and makes education or treatment recommendations.
Education or treatment
You complete the recommendations made for you. The required work and timing vary.
Follow-up evaluation
The SAP assesses whether you have successfully complied with the recommendations.
Return-to-duty testing
After the SAP determines successful compliance, an employer that intends to return you to safety-sensitive work must arrange the required return-to-duty test. The required negative drug test result and/or qualifying alcohol test result must be received before safety-sensitive duties resume. The employer still decides whether to return you to that work.
Follow-up testing
If you return, the employer implements the SAP’s follow-up testing plan and any applicable ongoing requirements.

More about the processWhat is an SAP?
A Substance Abuse Professional (SAP) evaluates employees who have violated DOT drug and alcohol rules and recommends education or treatment. The SAP also assesses compliance with those recommendations.
Will I receive three quotes?
You can request up to three quotes. The number of providers able to respond depends on your location, appointment preferences and availability. Three quotes are not guaranteed.
Is DOT SAP Program a treatment provider?
No. This is an independent quote-request service. SAP providers carry out their own evaluations, set their own fees and are responsible for the services they deliver.
Can an evaluation take place online?
A provider may offer a remote evaluation when it meets DOT requirements and their professional credential permits it. Confirm eligibility, location requirements and appointment format directly with the provider.
Does completing the SAP process guarantee a job?
No. The SAP process, required testing and the employer’s decision are separate. A quote or SAP evaluation does not guarantee return to safety-sensitive work or employment.
